Patient Responsibilities

Payment Terms
Payment is expected at the time of service unless other arrangements have been made. By receiving services, you agree to the following payment terms:
- Co-payments and deductibles are due at the time of service
- You are responsible for understanding your insurance coverage and benefits
- If your insurance denies a claim, you are responsible for the balance
- Missed appointments without 24-hour notice may incur a fee
- Outstanding balances may be sent to collections after 90 days
