What is Serotonin Syndrome?
Serotonin syndrome is a potentially life-threatening drug reaction caused by too much serotonin accumulating in the brain. It typically occurs when medications that increase serotonin are combined or when starting a new serotonergic medication before another has cleared the system.
Mild to Moderate Symptoms
- Agitation or restlessness
- Rapid heartbeat (tachycardia)
- Dilated pupils
- Heavy sweating
- Diarrhea
- Muscle twitching
Severe Symptoms (Call 911)
- High fever (over 104°F / 40°C)
- Seizures
- Irregular heartbeat
- Severe muscle rigidity
- Loss of consciousness
- Extreme agitation or delirium
The Classic Triad of Serotonin Syndrome
Mental Status Changes
Agitation, anxiety, restlessness, confusion
Autonomic Instability
Fever, sweating, rapid heart rate, blood pressure changes
Neuromuscular Abnormalities
Tremor, muscle rigidity, hyperreflexia, clonus
Common Causes of Serotonin Syndrome
- Taking multiple serotonergic medications together
- Starting a new SSRI or SNRI before the old one clears
- Combining antidepressants with certain migraine medications (triptans)
- Taking MAOIs with other antidepressants
- Combining prescription medications with St. John's Wort
- Using illicit drugs like MDMA (ecstasy) with antidepressants
- Overdose of serotonergic medications

What To Do If You Suspect Serotonin Syndrome
For Severe Symptoms
Call 911 immediately. Stop taking all serotonergic medications. This is a medical emergency requiring hospital treatment.
For Mild Symptoms
Contact your prescribing psychiatrist or call our office immediately at 678-437-1659. Do not take additional doses of your medication. We may need to adjust or stop certain medications.
