Bipolar II disorder is characterized by episodes of major depression alternating with hypomanic episodes—a milder form of mania. Hypomania can feel euphoric or productive, making people reluctant to seek treatment. But hypomanic episodes often precede depressive crashes and carry their own risks. What Is Hypomania?

Hypomania involves elevated, expansive, or irritable mood lasting at least four consecutive days. Unlike mania, hypomania doesn't typically cause psychotic symptoms or severe functional impairment, but it represents a clear change from your baseline mood and functioning.

Increased Energy and Activity

You feel more energized, productive, and driven than usual. You accomplish more, feel less tired, and may take on new projects with enthusiasm. This can feel good and lead to avoidance of treatment.

You feel rested after just a few hours of sleep. This is different from insomnia—you feel genuinely refreshed despite sleeping less. This sleep shift is often the earliest warning sign.

Racing Thoughts and Fast Speech

Your mind feels quick and nimble. Thoughts come rapidly, you talk faster, and you jump between topics. Conversations can feel effortless and stimulating.

Increased Confidence and Goal-Directed Activity

You feel capable, confident, even invincible. You may start ambitious projects, make major life changes, or take on significant commitments.

Increased Impulsivity

You make purchases, relationship decisions, or risky choices with less deliberation. Spending increases. You might start new relationships or make significant job changes.

Sociability and Distractibility

You feel more social and talkative, enjoy big social plans, and may initiate more activities. Conversely, you might find it hard to focus on boring tasks.

Irritability (Often Overlooked)

Not all hypomanic episodes feel euphoric. Some people experience irritability, impatience, and low frustration tolerance during hypomania.

Why Hypomania Is Often Missed

Hypomania feels good. People are productive, social, and confident. They don't recognize it as a symptom, so they don't report it to their doctor. Many people are only diagnosed with bipolar II when the depressive episodes bring them to treatment.

The Hypomania-Depression Cycle

Without treatment, hypomania often transitions to depression. The increased spending, relationship changes, and commitments made during hypomania can create stress and guilt during subsequent depression, making the crash more severe.

If you experience distinct periods of elevated or irritable mood lasting several days, with decreased need for sleep, racing thoughts, or increased goal-directed activity, bipolar II evaluation is appropriate.
